Cognitive Biases in Children and Adolescents With Chronic Pain: A Review of Findings and a Call for Developmental Research

Cognitive biases that emphasize bodily harm, injury, and illness could play a role in the maintenance of chronic pain, by facilitating fear and avoidance. Whereas extensive research has established attention, interpretation, and memory biases in adults with chronic pain, far less is known about these same biases in children and adolescents with pain. Studying cognitive biases in attention, interpretation, and memory in relation to pain occurring in youth is important because youth is a time when pain can first become chronic, and when relationships between cognitive biases and pain outcomes emerge and stabilize.

Provoked vestibulodynia (PVD) is a chronic pelvic pain disorder affecting approximately 16% of women,37 with a significant effect on quality of life.37 Individuals with PVD report localized hypersensitivity and severe pain of the vulvar vestibule upon vaginal penetration (eg, intercourse, tampon use) in the absence of detectable infection or inflammatory etiology.4,31 The underlying pathophysiology of PVD is largely unknown, and treatment options remain unsatisfactory.85 Available evidence suggests that peripheral as well as central alterations in sensory processing appear to be involved.

Complex regional pain syndrome (CRPS) may develop as a severe complication after limb trauma or lesions of the peripheral nervous system. One primary feature of CRPS is “continuing pain […] disproportionate to any inciting event”.26 Additional possible characteristic symptoms include sensory (hyperesthesia, allodynia), vasomotor (asymmetric temperature or skin color), motor (dysfunctions, restriction), and trophic changes (altered growth of skin appendages), edema, and changes in sweating of affected extremities.

Chronic pain conditions occurring in the lower abdomen and pelvis are prevalent, often challenging to manage, and can negatively affect health-related quality of life, including psychosocial and sexual well-being.3,8,25,27,32,37 Very few U.S. Food and Drug Administration (FDA) or European Medicines Agency (EMA) approved treatments currently exist for these conditions. The dearth of treatments arises, in part, from methodological challenges in designing randomized clinical trials (RCTs) to evaluate the efficacy of novel treatments in these conditions.

Pain intensity represents the primary outcome in most clinical trials of pain disorders and is nearly universally assessed in chronic pain research.32,79 Chronic pain affects over 11% of the population of the United States,90 and there is an undisputed need for the accurate and reliable assessment of pain. Although alternatives to self-reported pain intensity (eg, observation of pain behaviors64) have previously been considered, self-reports presently constitute the gold standard of pain assessment because they are able to reflect the subjectivity inherent in the pain experience.

A Kappa Opioid Receptor Agonist Blocks Bone Cancer Pain Without Altering Bone Loss, Tumor Size, or Cancer Cell Proliferation in a Mouse Model of Cancer-Induced Bone Pain

Tumor growth within bone is associated with moderate to severe pain. Pain is the most feared consequence of cancer and greatly diminishes the patient’s quality of life.5,34 Mu opioid receptor (MOR)-mediated drugs such as morphine and fentanyl remain the gold standard for the treatment of moderate to severe cancer pain including ongoing and breakthrough cancer pain.1 However, the severity of cancer pain generally requires high doses, and sustained treatment leads to tolerance, which leads to dose escalation.
